A Medical Release Form must be on file for all children in the program. This form must be notarized. A nortary is provided at the church if needed.

Children's Market Car Seat/Infant Carrier Waiver

Car seats must be less than 5 years old and cannot be sold if they have been in an accident with or without a child in the seat. Please check for recalls before bring a car seat or carrier to the sale.
